How do I request a police report?
It is best to make your requests online, click here.
You may also make a request in person at the Clearfield City Customer Service Center located at 55. S State Street in Clearfield Utah.
What are the fees?
City Records
- First 15 minutes at no charge
- Assessed at the cost of the hourly rate of the staff member who has the necessary skills and training to perform the request.
Police Records
- Written Report & Attachments - $16 / report
- Body or Dash Camera Video - $75 / hour of video
- Motor Vehicle Accident Report (police report, DI-9, & attachments) - $25 / report
- Victims of domestic violence, sexual assault, & stalking have no fees for reports
Justice Court
- Copies - $0.25 per page
- Certification - $4 + $0.50 per page
- Audio Recording - $15
- Research:
- 1-15 minutes - No Charge
- 15+ minutes - $15 - $21 per hour (lowest rate of employee able to fulfill request)
How do I receive the police report
You have several options to receive your request. You may receive the request through email, in-person, or have it mailed directly to you.
How do I appeal the record's officer decision?
You may appeal the determination within 30 days from the date of this letter by filing written notice with the office of the City Manager, 55 South State Street, Clearfield, Utah, 84015. The notice of appeal shall contain the petitioner's name, address, phone number, relief sought and shall set forth in detail a statement of the facts, reasons and legal authority relied upon in making the appeal in accordance with City Code ยง 1-10-15(A).
